For reasons psychological or physiological, you have a perpetual case of the shakes. Some days it's bad and some days it's barely noticeable, but it never goes away. Drugs may suppress the condition, but the shaking always returns. You suffer a 1D penalty on all Dexterity actions, and a 2D penalty on fine manipulation--you'd be a terrible surgeon.
